Partnership for Healthcare Payment Reform (PHPR)

WCHQ supports the Partnership for Healthcare Payment Reform (PHPR), which is piloting new payment models to improve the quality and affordability of healthcare in Wisconsin and advance the health status of Wisconsin residents.
PHPR convenes Wisconsin's diverse healthcare stakeholders to assess, design, test and implement innovative, comprehensive approaches to healthcare payment reform. PHPR partners, including WCHQ, recognize that a collaborative approach to healthcare payment reform allows for the alignment of incentives among payers, maximizes purchasing strategies, allows for shared learning and analysis, and builds on Wisconsin's healthcare assets and commitments to quality and value.
PHPR Total Knee Replacement Bundled Payment Pilot
Total Knee Replacement (TKR) is an increasingly common acute care procedure with costs that frequently vary from provider to provider. PHPR launched an acute care pilot program for TKR, because not only is it a common surgical procedure but evidence also strongly suggests that bundled payment could improve both the quality and efficiency of TKR care.
PHPR's bundled payment pilot program covers the cost of the TKR procedure and post-operative care for qualified patients. Additionally, it includes a 90-day warranty to cover complications or device failure.
Three pilot program organizations -- Manitowoc Surgery Center, Meriter Hospital, and UW Hospital and Clinics -- have been providing quality data on their TKR patient population for analysis. WCHQ took the lead in preparing a report of TKR quality results from 2013, the pilot program's first year, in concert with all pilot participants, the Wisconsin Health Information Organization, and Wisconsin Hospital Association. This report presents quality data using multiple measures, such as readmission and complication rates, patient experience, and patient reported outcomes.
