WCHQ has aligned with the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness and Data Information Set (HEDIS) specifications for measuring A1C good control for patients with diabetes. The measure separates patients with certain co-morbid conditions and considers an A1C of less than 8.0% good control. The remaining patients that do not fall into the co-morbid conditions category have an A1C goal of less than 7.0%. The co-morbid conditions include:
- Patients aged 65-75
- Coronary Artery Bypass Graft (CABG) or Percutaneous Transluminal Coronary Angioplasty (PTCA)
- Ischemic Vascular Disease
- Heart Failure
- Myocardial Infarction
- Chronic Renal Failure
- Dementia
- Blindness
- Amputation of Lower Extremity
This measure assesses the following:
- Good Control - A1c less than 7.0% (excludes patients aged 65-75 or those with the above listed co-morbid conditions)
- Good Control - A1c less than 8.0% (includes patients aged 65-75 or those with the above listed co-morbid conditions)
* All patients with an A1C of less than 7.0% and less than 8.0% are included together in good control
- Fair to Poor Control - A1c greater than or equal to 7.0% and less than or equal to 9.0% or greater than or equal to 8.0% and less than or equal to 9.0% for patients aged 65-75 or those with the above listed co-morbid conditions
- Uncontrolled - A1c greater than 9.0%
- No A1c test within the measurement period
