The Obesity Toolkit
The WCHQ Obesity Advisory Group created this toolkit to address both clinical and operational workflows for treating obesity in primary care practice. WCHQ members identified the chronic disease of obesity as a growing concern, and in response, developed measures, education and improvement activities. This has been a vital step to improve treatment of the disease. The group produced this toolkit to provide relevant information for both the clinical provider and the operational administrator to build a successful obesity treatment workflow.
The impact of obesity on the workplace is significant. Studies have shown that significant impairment at work is often related to obesity. Employers can play a role in helping employees who have obesity.

Chronic Kidney Disease (CKD) Toolkit
The objective of this toolkit is to encourage primary care practices to adhere to standards of care in screening for CKD, understand diagnosis of CKD, manage and treat CKD in their practice, and refer to nephrology when necessary. This toolkit is a practical, usable tool for quick referencing and following standards of care. The final workflows met the latest evidence-based, updated clinical guidelines and care models from professional medical societies and associations for screening for and treatment of CKD.

Childhood and Adolescent Immunization Toolkit
The Childhood and Adolescent Immunization Toolkit aims to empower members of the patient care team by providing a comprehensive and accessible resource that enhances their knowledge of immunization practices. This toolkit seeks to serve as a valuable reference guide, equipping healthcare professionals with the necessary information to educate themselves on current immunization recommendations, guidelines, and resources.

Health Disparities Improvement Toolkit
Wisconsin has led the way nationally in improving health care quality. However, there are widespread disparities in health outcomes and care. The objective of this toolkit is to share resources that health system and community partners utilize. The resources we have collected and proven to be helpful are then cataloged in one central place. We encourage you to utilize these resources throughout your work as you work to improve care and decrease disparities in health outcomes throughout Wisconsin.

Maternal Health Toolkit
Maternal health care is foundational to healthy children, healthy families, and healthy communities across Wisconsin. Despite ranking high in industrialized nations, maternal mortality and morbidity rates have steadily increased in the US over the past 20 years. Elevating conversations revealing the data, bringing together individuals working to solve the problem and listening to mothers and their families must be coupled with tools to address the problem at the influential level. This toolkit is an attempt to provide resources to expand access to information that will help address the issue with the hope of reducing maternal morbidity and mortality rates in Wisconsin.

This platform highlights powerful stories shared directly by patients, offering unique insights into their healthcare journeys. By amplifying these voices, we aim to inspire meaningful change, helping providers better understand and address patient needs while driving quality improvement across Wisconsin. We offer discussion guides for each set of brief videos for training purposes and staff onboarding to further improve care.
