MADISON, Wis., October 14, 2025 — The Wisconsin Collaborative for Healthcare Quality (WCHQ) has been awarded a $250,000 Momentum Grant from the Advancing a Healthier Wisconsin Endowment (AHW) to accelerate efforts that reduce health disparities and promote health equity across the state.
The two-year project will use healthcare data to identify and share effective, evidence-based strategies that close gaps in care and improve outcomes for underserved populations. WCHQ will work with its statewide network of healthcare members to identify approaches already showing success in advancing health equity.
By analyzing WCHQ’s comprehensive data, including measures such as depression screening and diabetes management, the project will determine which interventions most effectively reduce disparities in care. Insights will be shared widely to help healthcare organizations across Wisconsin replicate proven strategies.

AHW Momentum Grants support research and community programs that improve health in Wisconsin. The funding, which begins November 1, 2025, covers eligible expenses over a 24-month period and is approved through a rigorous review process by the MCW Consortium on Public and Community Health, the AHW Research and Education Advisory Committee, and the MCW Board of Trustees.
