Cardiovascular Kidney Metabolic Toolkit

Hypertension (HTN), diabetes, obesity, and chronic kidney disease (CKD) frequently coexist and increase the risk of cardiovascular events, kidney failure, and premature mortality. Early screening and evidence-based treatment can significantly improve patient outcomes. This toolkit provides practical resources to support the identification, management, and referral of patients with cardiovascular-kidney-metabolic (CKM) syndrome in primary care settings.

The Objective

  • For providers, residents, fellows, and care teams working with adult populations to use
  • Practical reference resource to support evidence-based screening, diagnosis, and treatment
  • Determine when referrals are necessary for patients with CKM conditions
