Advancing Care for Cardiometabolic Disease Assembly Summary
August 1, 2025 Assembly Summary
On Friday, August 1, 2025 WCHQ brought together nearly 180 healthcare stakeholders from across Wisconsin for an assembly focused on Advancing Care for Cardiometabolic Disease. Attendees learned how collaborative, data-driven strategies can improve cardio-metabolic health by focusing on early detection, prevention, and holistic care. Additionally, they gained insights into new measures for diabetes, obesity, and kidney disease, the integration of pharmacists into care teams, and the value of transparent data sharing and public reporting. Real-world patient stories and research advances highlighted the need for multidisciplinary teamwork and continuous innovation to address gaps and improve outcomes.

Driving Quality Improvement in Healthcare: Achievements and Challenges Across Wisconsin
The WCHQ Improvement Advisory Committee (IAC) sets goals each year to advance the quality of care across Wisconsin. These goals guide our members’ improvement efforts, with a particular focus on improving outcomes for patients with chronic conditions, increasing preventive screenings, and ensuring equity in care.
Two areas in particular stand out in this year’s results: blood sugar (A1c) control and colorectal cancer screening.

13 of 18 members now meet the 75% threshold for blood sugar (A1C) control. This improvement resulted in more than 6,000 additional patients with controlled A1c.

Colorectal screening is recovering following the measure update to include ages 45–49. More than 6,400 additional patients were screened for colorectal cancer.
Looking ahead, WCHQ and its members will continue to use data-driven insights to target areas of greatest need, close gaps in care, and ensure that every patient has access to high-quality, equitable healthcare across Wisconsin.
CKD Risk Stratification and Care: Guidance from the National Kidney Foundation’s Chief Medical Officer
Join us on Thursday, October 2, 2025 for an engaging session with Dr. Joseph Vassalotti, Chief Medical Officer of the National Kidney Foundation and Clinical Professor of Medicine in the Division of Nephrology at the Icahn School of Medicine at Mount Sinai. Dr. Vassalotti is a nationally recognized leader in chronic kidney disease (CKD) prevention and management, with expertise in primary care, population health, lifestyle interventions, interdisciplinary care, and the use of kidney- and cardioprotective medications.
This session will explore how to:
- Identify who to test and which tests to order for CKD risk stratification.
- Interpret and apply measures of kidney function and damage, including eGFR and albuminuria.
- Integrate lifestyle modifications, nutrition, pharmacologic therapies, and interdisciplinary care into CKD management.
- Understand the relationship between CKD and cardiovascular disease through the lens of the Cardiovascular-Kidney-Metabolic (CKM) Syndrome.
Through case presentations and evidence-based strategies, Dr. Vassalotti will provide practical insights into diagnosing, risk stratifying, and intervening earlier in CKD to improve kidney and cardiovascular outcomes.

Advancing Healthcare Quality Through Strategic Partnerships
We’re happy to announce that the Wisconsin Collaborative for Healthcare Quality (WCHQ) has formed a strategic partnership with the Wellness Council of Wisconsin (WCWI), joining forces to advance both workforce well-being and healthcare quality across our state. This collaboration is rooted in a shared commitment to building a healthier Wisconsin through leveraging data-driven strategies, cross-sector collaboration, and a focus on the well-being of those who deliver and support care.
Through this new partnership, WCHQ and WCWI will collaborate on several priorities:
- Supporting mental health and well-being: Launching joint initiatives to promote mental health and resilience among healthcare and public health professionals.
- Integrating workforce well-being data: Exploring ways to include workforce well-being measures within broader healthcare performance strategies.
- Sharing resources and best practices: Providing tools and insights that enhance both population health and the strength of the healthcare workforce.
- Partnering on events: Co-hosting events and conversations that inspire action and facilitate cross-sector collaboration.
Partnering with WCWI allows us to expand our focus beyond clinical outcomes to include the well-being of those who make those outcomes possible. Together, we’re building a stronger, more sustainable foundation for health in Wisconsin.
We look forward to sharing updates and inviting our members to participate in upcoming initiatives and events.
