WCHQ Member Update: June 2025 Quality Results
We’re excited to share the latest results from our quality improvement efforts over the past year. Thanks to member dedication, thousands of patients across Wisconsin are experiencing better health outcomes. Here’s a look at what the Collaborative accomplished—and where we can work together to do even more.
Our Successes: Making a Real Difference
Diabetes Care: Over 4,500 more patients now have their diabetes under control, thanks to the Collaborative’s hard work and commitment to chronic disease management. Our statewide rate for optimal diabetes control rose to 50%, showing real progress in helping people live healthier lives.

Depression Screening: The collaborative screened more than 5,100 additional patients for depression, reaching a new high of 87%. Early detection means more people are getting the support they need.

New Measures for Better Care: This year, we began tracking new ways to support patients with diabetes and depression.
- We’re measuring how well we’re testing for kidney disease in people with diabetes (68% of patients received recommended testing)
- We’re examining how patients respond to depression treatment over time (8% of adolescents and 15% of adults achieved depression remission six months from the time of diagnosis).
Opportunities to Improve: Let’s Tackle These Together
Immunizations: While adolescent immunizations improved, we saw small declines in childhood and adult vaccinations. HPV vaccination rates held steady, which is a bright spot, but we know there’s more we can do to protect our communities.
Tobacco-Free Status: Fewer patients with heart disease and diabetes are reporting tobacco-free status. This is an area where we can renew our focus and share ideas to help more people quit for good.
